As a rough approximation, you can fit a sphere to the inner skull surface (created, e.g., by the watershed) as we did in some of the TMS modeling stuff (see, appendix of):

and then scale the result with an approximation of a brain-to-skin distance.

Or you could: 
1) Use "mkheadsurf" to create a head surface. 
2) Pick two anatomical landmark points such as nasion and inion and use the Dijkstra algorithm to
    calculate the length of the shortest path (the Dijkstra may require using the FreeSurfer  MATLAB toolbox). 
    between those points.
