I don't know the internal details for FreeSurfer, maybe others can help with those. But in general, the vertices and faces are constructed *together* from the (pre-processed) image voxels by algorithms like Marching Cubes:

There are many possibilities and options for mesh construction though, the relevant field is called Computer Graphics.
