This might be the bug that was fixed in freesurfer v4.0.1. The workaround for this (if not using the tkmedit in v4.0.1) is to load the talairach.lta file instead (or use tkregister2 and load talairach.xfm, as you have discovered).

Ohps! Sorry, it is the talairach.xfm file loaded in tkmedit as explained on the tutorial page.
