Tim,
You can do the following, assuming you have already installed X Windows, and have configured your .cshrc file to setup FREESURFER_HOME (which for the Mac install is the directory /Applications/freesurfer).
Open an X Terminal window, and type:
cd $FREESURFER_HOME pico .license
then copy and paste the three lines of your license text into the file. Then Ctrl-X to save. The 'pico' application is a very simple text editor that does not make any assumptions based on the filename extension.
